What happens when my spouse gains their own benefits plan?

If your spouse gains their own benefits plan, the following steps must be taken:

• If you wish to change your PEBT Benefits Program coverage, you must complete the PEBT Benefits Change Form.
• If you wish to waive extended health and/or dental coverage under the PEBT Benefits Program and join your spouse’s plan, complete the Waiver of Coverage Form. Refer to the What happens when I want to waive coverage for myself and/or my dependents section for more details.
