Advise the Health Care Management Specialist (HCMS) at Desjardins and the Union JEIS Coordinator when members have been, or are expected to be, absent for six consecutive scheduled working days/shifts.

Provide basic information to HCMS and the Union JEIS Coordinator (name, phone number, address, last date worked, etc.) as required.
