Transition to LTD
The LTD application process
Sometimes a member’s disability will keep them away from work for a longer period of time. Click below to learn about the steps each role is responsible for during the transition to LTD phase.

After eight weeks from the date of disability, evaluate if the member’s file should be forwarded to the LTD department at Desjardins.
If it is very likely the member will safely return to work prior to the LTD effective date, defer sending the file and monitor to ensure the plan remains on track and expectations have not changed.

At least six weeks in advance of the LTD effective date, send a copy of the member’s file to the Desjardins claims adjudicator to allow the claims specialist enough time to review the file and request any further information required.
Once all the required information is on file (e.g., Member Authorization, Attending Physician Statement, Initial Employer Report), and a telephone interview with the LTD claims specialist is completed, an LTD claim decision will be provided within 10 business days.
Together with the claims specialist discuss the member’s file for ease of transition.

For member rehabilitation programs that will continue for 2-4 weeks into the LTD period, remain involved to ensure the transition is smooth and the member continues to feel supported.

Advise: Advise the HCMS if a member returns to work prior to transitioning to LTD.

Notify: Ensure that the HCMS is promptly notified when a member is not able to sustain a return to work based on their regular duties and hours.
3.
Meet with the Member: When a member is transitioning to LTD, reach out to the member to discuss what type of contact they would feel comfortable with from the workplace and/or Union (e.g., emails, phone calls, newsletters, invites to events, etc.) on an ongoing basis.

Advise the HMCS or the LTD Claims specialist of any medical or personal updates that will potentially impact their LTD claim. This includes reporting any additional income, participation in work, education, or volunteer activities, as well as any plans to travel outside of Canada.
