School district submits referral on day 6 or earlier

Who is eligible for JEIS

  • Regular unionized support staff working 15+ hours per week for at least 3 consecutive months

  • Includes regular employees in temporary assignments

  • Does not include casual or non-regular temporary employees

  • No age restrictions — eligibility applies regardless of age

When to send a JEIS referral

  • Refer when the member has missed or is expected to miss 6 consecutive scheduled workdays due to illness or injury.

  • Send right away if absence is expected to last 6 days—even if it’s only Day 1.

  • If sending the referral before day 6, include an explanation of why in the comments section of the form.

  • Do not refer if the member:

    • Is still actively at work
    • Is working part-time or on modified duties
    • Returns before missing 6 consecutive scheduled days
  • Timely JEIS referrals—ideally made as close to the 6th day of absence as possible—are linked to better return-to-work outcomes. Check your district’s past JEIS notifications times.

  • If a referral is late or has been missed – please send as soon as possible. If a referral is over 30 days old, provide an explanation for the delay in the comments section of the referral form.

How to send a JEIS referral

  • Complete the JEIS Referral Form

    • “Effective Date of Coverage” = member’s Core LTD eligibility start date (after 3 months of active employment)
    • Ensure the “Last day worked” is completed (the last day the member was physically at work)
    • Use the ‘Comments’ section thoughtfully. Include information that helps Desjardins provide appropriate, compassionate support, such as:
      • Recent loss, trauma, accident, surgery, or hospitalization
      • Return-to-work plans or availability
      • Workplace conflict or leave status (e.g. suspension)
  • Submit the Referral to Desjardins
