Capture what helped, what hindered, and how to improve future RTWs — without adding pressure on the returning employee.

How to Collect Feedback

  • Timing: 1–2 weeks after the RTW plan has been completed or ended early.

  • Format: 5-minute conversation, short email, or 2-question anonymous form.

  • Tone: Supportive and improvement-focused — not evaluative.

  • Reassurance: Clarify that answers are used to improve the RTW process.

How to Use the Feedback

  • Share key insights with your Complex HCMS during your monthly District JEIS meetings.

  • Use the information to support continuous improvement by identifying both challenges and successes from the perspectives of members and JEIS Committees.

Suggested Talking Points

  • “Hi [Name], now that you’ve been back for a few weeks, we’re checking in to see how your return-to-work plan has felt. This is just to help us improve support for employees returning to work — not to evaluate performance. Would you be open to a quick 5-minute check-in or completing a short feedback form?”

  • “Everyone’s experience is different, so your insights really help us understand what worked well and what could be smoother next time.”
